DENTAL INSURANCE GUIDELINES

One of the many benefits of Chamber membership is the availability of competitively priced group dental insurance. We currently offer Dental Pay Plus coverage, providing the flexibility of seeking both in and out of network care.  Each enrolled employee will receive an ID card with a summary plan booklet detailing the coverage.  Children are covered to age 19 or until age 23 if a full-time student carrying at least 12 credit hours per semester.  You will be billed quarterly (December, March, June and September) by the Chamber for your coverage.  You will be responsible for paying your dentist directly for any co-payment required.  Claim forms for non-participating dentists are online or available in the Chamber office.  You will receive a statement from Dental Pay Plus detailing each claim submitted and processed.

The following guidelines apply:

  • Only business members and/or their employees who work at least 20 hours per week are eligible for coverage.  If it is found that an ineligible employee is allowed to join, the entire group may be cancelled.
  • Effective date for new eligible chamber members will be the first of the month following enrollment.  This is predicated on your application being received by the 20th of the preceding month.
  • You must remain enrolled in the plan for at least 12 months.  If you cancel coverage at the end of the 12-month period, you must wait 24 months until you are again eligible for coverage.
  • New Chamber business members and their current employees may enroll within 30 days of joining the Chamber.  Coverage will be effective the 1st of the month following the 30-day period.
  • New employees of enrolled companies may enroll within 30 days from their date of hire, or within 30 days of eligibility.
  • Retirees are eligible for enrollment in the Chambers’ dental insurance program only if they were employed by a Chamber member and in the dental plan prior to retirement.
  • Married couples can apply for two single policies only if they can document that both are employees of a Chamber-member business and both work at least 20 hours per week.  If not, they must enroll for a family policy.
  • Below are the quarterly premiums for the plan offered.  You must remain in the plan in which you enroll in until next open enrollment.  The Chamber bills members on a quarterly basis.  Please note these rates do not include the administrative fee charged by the Chamber to administer the dental plan.
